*Please download the Health Screen and Permission Form and answer ALL the questions and fill in EVERY box (the form must be downloaded for all the information to appear on the form; if it is not downloaded when you try to print it, important information will be missing).  If the form is not completely filled in, it will need to be returned to you and this may delay vaccine administration.  Insurance does not get charged for this vaccine,  since this is from the State, but the information is needed to input data into the Maine Immunization Registry.

    *Send the completed permission form to school with your child by OCTOBER, 26TH.  We will NOT be able to accept any permission forms after that date due to vaccine ordering that needs to be done beforehand.
